Product Overview Tramadol Hydrochloride 100 mg is a prescription opioid analgesic in extended-release formulation. It is used for the management of moderate to moderately severe chronic pain in adults that requires around-the-clock treatment for an extended period, when alternative pain medications are inadequate or not tolerated. Tramadol works by altering how the brain perceives pain, through opioid receptor binding and inhibition of norepinephrine and serotonin reuptake.

Important Warning Tramadol is a controlled substance with high potential for addiction, abuse, misuse, overdose, and death. Risk is increased with concurrent use of alcohol or other CNS depressants. Life-threatening respiratory depression can occur, especially during initiation or dose increases. Swallow whole—do not crush, chew, split, or dissolve, as this can release the full dose rapidly and cause fatal overdose. Accidental ingestion (especially by children) can be fatal. Prolonged use during pregnancy can cause neonatal opioid withdrawal syndrome. Always follow your doctor’s instructions precisely and discuss naloxone availability for overdose reversal.

Dosage and Administration

  • Adults (opioid-naive): Start with 100 mg once daily. May increase by 100 mg every 5 days as needed and tolerated.
  • Maximum dose: 300 mg per day.
  • Patients switching from immediate-release tramadol: Calculate 24-hour dose and round down to nearest 100 mg increment for once-daily dosing.
  • Take with or without food, but consistently. Swallow whole.
  • Elderly or patients with renal/hepatic impairment: Lower doses or avoid extended-release form; consult doctor. Not recommended in severe renal (CrCl <30 mL/min) or hepatic impairment.
  • Do not abruptly stop—taper gradually under medical supervision to avoid withdrawal symptoms.

Common Side Effects

  • Dizziness, headache, or drowsiness
  • Nausea, vomiting, or constipation
  • Fatigue, dry mouth, or itching
  • Sweating or vertigo

Serious side effects (seek immediate medical help): Slow/shallow breathing, severe drowsiness/confusion, seizures, serotonin syndrome (agitation, hallucinations, rapid heartbeat), low cortisol levels (nausea, loss of appetite), or allergic reaction (rash, swelling).

Precautions and Contraindications

  • Do not use if you have: Severe respiratory depression, acute or severe bronchial asthma, known hypersensitivity to tramadol/opioids, recent MAOI use (within 14 days), or gastrointestinal obstruction.
  • Avoid alcohol and other CNS depressants (e.g., benzodiazepines, sedatives, other opioids)—can cause life-threatening respiratory depression or sedation.
